Registration System Forms

Please submit all completed forms via email to . Forms for new events are due 7 days before an event in order to guarantee quality of service.

 

 

CUS Events

  • CUS Event Form - Use this form to set up a new CUS event. This will set up the basic event, so read the bottom of the form for more information on setting up advanced features.

Business Clubs

  • Club Membership Form - Use this form to set up membership for the club. This will set up a basic registration form, so read the bottom of the form for more information on setting up advanced features.
  • Club Event Form - Use this form to set up a new event for club. This will set up the basic event, so read the bottom of the form for more information on setting up advanced features.

Miscellaneous Forms

  • Administrator Privilege Form - In order to manage your event, sell tickets or view people that have signed up, you will need administrator privileges for your event. Please complete this form to obtain the permissions that you need.
  • Event Confirmation Email Form - Customize the confirmation email that every registrant receives by writing your own text that includes more information about the event.
  • Event Specific Information Form - If you want to collect additional information besides name, email, year level, option and allergy information, fill out this form. You can choose from a variety of different fields such as checkboxes, drop downs, text boxes and more.
  • Multitrack Workshop Form - If your event has a series of concurrent workshops, fill out this form to allow students to register for specific workshops ahead of time.
